Reimbursement Request Instructions:
Tire Hazard Reimbursement:
You qualify for this reimbursement if any tire on your registered vehicle needs to be replaced because of a road hazard. This benefit does not apply to tires replaced due to normal wear and tear. To be reimbursed, please fill in the information requested on a claim form and mail to us with a copy of your paid invoice from the tire replacement facility. The receipt should show the vehicle year, make, model, and VIN # of your vehicle.
Emergency Roadside Assistance:
This benefit covers roadside repair such as a tire change, jump start, or gasoline delivery. Please fill out the information requested on the claim form and attach your receipt from the roadside assistance company that you used. The receipt must show your vehicle year, make, model, and VIN number.
Emergency Travel Expense:
In order to qualify for this reimbursement, your vehicle must have suffered a mechanical breakdown more than 100 miles from your home. To be reimbursed, please fill out the information requested from the claim form. Include your receipts for meals and your hotel. You must also include the receipt from the repair facility where your car was repaired. This receipt must show your vehicle make, model, and VIN number.
Lockout Reimbursement:
To be reimbursed, please fill in the information requested on a claim form and mail to us with a copy of your paid invoice from the lockout company. The receipt should show the vehicle year, make, model, and VIN # of your vehicle.
Ambulance Service Reimbursement:
You qualify for this reimbursement if you are involved in a collision in your registered vehicle requiring yourself or a family member to need an ambulance. To be reimbursed, please fill in the information requested on a claim form and mail to us with a copy of your paid invoice from the ambulance service. You must also include a copy of the policy report from the accident.
Exterior Reimbursement:
You qualify for this reimbursement if your registered vehicle requires replacement of headlights, taillights, fog lights, antennas, windshields, side mirrors, or hub caps. To be reimbursed, please fill in the information requested on a claim form and mail to us with a copy of your paid invoice from the repair facility. The receipt should show the vehicle year, make, model, and VIN # of your vehicle.
Emergency Towing Reimbursement:
You qualify for towing reimbursement if your registered vehicle breaks down and needs towing. To be reimbursed, please fill in the information requested on a claim form and mail to us with a copy of your paid invoice from the towing facility. The receipt should show the vehicle year, make, model, and VIN # of your vehicle.
Insurance Deductible Reimbursement:
To be reimbursed, please fill in the information requested on a claim form and mail to us with a copy of your police report and your insurance company’s paperwork for your insurance claim.
